It looks like the UV map covers the entire "texture" (body and stuff), here is a picture that shows the uv spaces that EA uses, for hats you should use the hat space (or any empty space if needed). i.imgur.com/kmJwdG6.png |

If you look at your second image, the texture is flipped vertically. What program are you using to make your textures and what format are you saving them in? This is a common issue if your settings are not correct when you save as a .dds using photoshop. If that's the case then first try saving as .png since studio will accept that format.

I'm not creating the textures, I extract them from Team Fortress 2 then covert it from Vtf to jpeg then from jpeg to dds in gimp. I save the dds file with BC3/DXT5 compression.
